How It Works
1. Request Your Prepaid Shipping Label
Place your order and provide your shipping information. We’ll email you a prepaid shipping label so you can send us your broken frame and at least one lens.
2. Send Us Your Lens & Frame
Pack your lens and broken frame securely and attach the prepaid label. Once received, we use your actual lens to create and fit your custom replacement frame for greater accuracy.
3. We Build & Fit Your New Frame
We manufacture your FormFix replacement frame specifically for your lenses and test the fit before it leaves our shop.
4. Receive Your Ready-to-Wear Glasses
We install your lenses into your new FormFix frame and ship your completed glasses back to you — no lens installation required.
What to Send
Your Broken Frame
Send the original frame whenever possible. It helps us preserve important measurements and positioning.
At Least One Lens
We need at least one original lens to accurately reproduce the lens shape and ensure proper fit. Sending both lenses is recommended so we can install and verify everything before returning your glasses.
Your Shipping Is Covered
After placing your order, you'll receive a prepaid shipping label by email. Pack your frame and lenses securely, attach the label, and drop the package off with the designated carrier.
